The study, published in the <em data-start=\"366\" data-end=\"401\">Journal of Clinical Investigation<\/em>, shows that mechanical strain \u2014 such as that occurring during mechanical ventilation \u2014 can significantly worsen bacterial lung infections.<\/p>\n<p data-start=\"538\" data-end=\"1127\">Ventilator-associated pneumonia is among the most common infections in intensive care units and is associated with high mortality. However, its pathophysiology remains insufficiently understood, partly due to the lack of suitable models. To address this gap, the researchers developed a so-called \u201cpneumonia-on-a-chip\u201d system. This system is based on a microfluidic lung model that mimics the alveolar-capillary interface, air\u2013liquid transitions, and the mechanical stretching movements of breathing. Primary human lung cells were used, including alveolar epithelial and endothelial cells.<\/p>\n<h3 data-start=\"1129\" data-end=\"1167\">Mechanical strain as a risk factor<\/h3>\n<p data-start=\"1169\" data-end=\"1465\">The key finding: Increased mechanical strain, comparable to more intensive ventilation, made lung tissue more susceptible to infection with <em data-start=\"1307\" data-end=\"1331\">Pseudomonas aeruginosa<\/em>, a typical pathogen in VAP. Under higher strain, the cells exhibited an altered immune response and increased bacterial colonization.<\/p>\n<p data-start=\"1467\" data-end=\"1642\">The model was also able to reproduce important disease processes, including bacterial colonization and virulence mechanisms that are critical for the progression of pneumonia.<\/p>\n<h3 data-start=\"1644\" data-end=\"1692\">Relevance for research and clinical practice<\/h3>\n<p data-start=\"1694\" data-end=\"2024\">The study provides important evidence that mechanical factors, particularly ventilation parameters, can actively contribute to the worsening of lung infections.
